You must be at least 18 years old and eligible for Medicare (or helping someone who is eligible) to use our Service. By using the Service, you represent and warrant that:

UniHealth provides assistance with Medicare enrollment, including:

• Submission of enrollment applications to Medicare/CMS
• Provider network verification (we may call doctor's offices to verify they accept specific plans)

No Guarantees

While we strive to provide accurate information and assistance:

• We do not guarantee enrollment in any specific plan
• We do not guarantee that all information is current or complete
• Medicare plan availability, benefits, and networks may change
• Final enrollment decisions are made by Medicare/CMS and insurance carriers

Your Responsibility

You are responsible for:

• Reviewing all enrollment information before submission
• Verifying that enrollment information is accurate
• Confirming enrollment with Medicare/CMS
• Notifying us of any errors or changes

SMS and Phone Communications

By providing your phone number and opting in, you consent to receive:

• SMS/text messages about enrollment status, reminders, and important updates
• Automated phone calls for provider network verification (to doctor's offices)
• Phone calls from us regarding your enrollment (with your consent)

Message and data rates may apply. You can opt-out of SMS by replying STOP. You can opt-out of phone calls by contacting us or updating your preferences.

Email Communications

We may send you emails about:

UniHealth provides information and assistance with Medicare enrollment. We do not provide:

• Medical advice or diagnosis
• Legal advice

• Insurance agent services (we are not licensed insurance agents)

Limitation of Liability

To the maximum extent permitted by law:

• UniHealth shall not be liable for any indirect, incidental, special, consequential, or punitive damages
• Our total liability shall not exceed the amount you paid us in the past 12 months (if any)
• We are not liable for enrollment decisions made by Medicare/CMS or insurance carriers
